Institute at OSF Saint Francis Medical Center is located
on the second floor of the Glen
Oak Building within the medical center complex. It is a designated 26 bed unit.
The unit serves patients with referrals from throughout the region.
The unit
serves individuals 13 years of age and older who would benefit
from rehabilitation nursing services and interdisciplinary rehabilitation
programming.
Rehabilitation is provided to patients who meet the admission criteria and
who are diagnosed with a variety of neurological and musculoskeletal disorders.
The
unit is under the direction of a physician who specializes
in physical medicine and rehabilitation (physiatrist) and
a nursing patient care manager.
A clinical
rehabilitation nurse specialist is available to patients to provide in-depth
expertise in rehabilitation.
Patients
who qualify for admission to the rehabilitation unit receive
a minimum of 3 hours of coordinated
therapy daily, a comprehensive evaluation
by all
needed services, and a coordinated treatment plan which centers on patient
and family
needs and concerns. The rehabilitation team meets weekly to update treatment
plans and ensure that the team focus remains on identified goals.
The
